You can use this function in your application to add keys to indexed array.

public static function convertIndexedArrayToAssociative($indexedArr, $keys)
{
    $resArr = array();
    foreach ($indexedArr as $item)
    {
        $tmpArr = array();
        foreach ($item as $key=>$value)
        {
            $tmpArr[$keys[$key]] = $value;
        }
        $resArr[] = $tmpArr;
    }
    return $resArr;
}
